We are seeking a registered nurse with at least 1 year of related experience to serve as a Case Manager in our Crisis Triage Center. In this role, you will coordinate the care team, manage treatment plans, and ensure quality, efficient care for our patients. You'll conduct assessments, develop care plans, and evaluate patient outcomes to ensure adherence to protocols and policies. You'll collaborate with physicians, nurses, and other resources to ensure continuity of care, facilitate communication with patients and families, and provide education as needed.

We have a Clinical Advancement Program and depending on your Nursing Education and National Certifications, you'll work towards one of five CAP levels that offer increasing compensation. You may earn up to $8.00 in addition to base pay. We also offer a sign-on bonus and relocation reimbursement to qualified candidates.

    Key Responsibilities:
  • Perform patient assessments and collaborate with care team to develop care plans
  • Coordinate with physicians and other resources to manage patient treatment plans
  • Evaluate patient outcomes and make adjustments to care plans as needed
  • Ensure continuity of care and effective communication with patients and families
  • Provide education and support to patients and families as appropriate

Qualifications:

  • Registered Nurse in NM or as allowed by reciprocal agreement by NM
  • CPR for Healthcare/BLS Prov or Prof Rescuers w/in 30 days
  • Graduate of a nationally accredited nursing program
  • Bachelor's Degree in nursing (preferred)
  • Bilingual English/Keres, Tewa, Tiwa, Towa, Zuni, or Navajo (preferred)

Note: Physical demands are in excess of those for Sedentary Work and may require walking or standing to a significant degree.
