Key Responsibilities:

  • Conduct comprehensive health assessments, screenings, and diagnostic evaluations to identify health concerns and develop individualized care plans.
  • Provide evidence-based treatment and management of acute and chronic health conditions, including prescribing medications, ordering diagnostic tests, and referring patients to specialty care as needed.
  • Educate patients and their families about their health conditions, treatment options, and preventive care strategies to empower them to take an active role in their healthcare.
  • Collaborate with interdisciplinary healthcare teams to coordinate patient care, facilitate referrals, and ensure continuity of care across healthcare settings.

Requirements:

  • Master's degree in Nursing, Physician Assistant Studies, or related field (required).
  • Current unrestricted license or certification as a Nurse Practitioner (NP), Physician Assistant (PA), or other advanced practice healthcare professional in the state of practice.
  • Minimum of 2 years of clinical experience in primary care or a related specialty.
  • Strong clinical assessment, diagnostic, and treatment skills.
  • Excellent communication, interpersonal, and problem-solving abilities.
